Oops! Note
I got this idea from Kim Adsit a few years ago when I saw her at Frog Street's Summer Splash. I have used it with my kids ever since. I use a traffic light behavior system so the note works perfectly.

Kim's "Red Face Note" or "Oops Note" can be downloaded from Kim's web-site. It is available in both Word (you will either need DJ Inker's fonts or substitute a cute one of your own) abd in .PDF form. The link to her letters are her:

This is FANTASTIC! I just came across this today and it would go perfectly with my behavior plans as well. I love this b/c I can print off multiple copies, keep it on my clipboard, and have documentation on each child's behavior daily this way!
